What controls the fan operation, is it just a thermal sending unit at a designated temp? Or does the ecu control it and can it be changed?

If it is a factory fan it is controlled by a temp switch operated relay. The set temp is 65 degC, so it will seem like it runs all the time. It can't be changed as far as I know.

Did you really mean to type 65c?? That's like 149degrees. Seems very low, I thought it would be closer to 85/90c

Yup, 65 degC is the set point.
